What information must I provide for creating a CPA account with Tax 990?

Please provide the following information while creating a CPA account with Tax 990.

If you own or work for a Firm

• Details of the firm you own or work for; including name and address of the firm
• Basic details such as your name, EIN, Preparer tax identification number (PTIN) (If you don't have PTIN, provide your SSN)

If you are self-employed

• Details such as your name, EIN, Preparer tax identification number (PTIN) (If you don't have PTIN, provide your SSN), and address.
