What is a conformed copy?

A conformed copy is a copy that agrees with the organization's original document and all its amendments. If the copy isn't signed, the organization must attach a written declaration signed by an officer who is authorized to sign on behalf of the organization, certifying that the copy of the original document is complete and accurate. 

Reporting information about conformed copy on Form 990-PF 

If your organization's governing document, articles of incorporation, bylaws, or other similar documents have undergone any modifications that have not yet been disclosed to the IRS, the changes must then be reported on Form 990-PF, Part VI-A - Statements Regarding Activities, Line 3, along with a conformed copy.
